Insurance And Payment Information

We are a provider for Medicare and most major insurance plans. It is the responsibility of the patient to know their insurance plan/coverage. We encourage all patients to confirm their coverage prior to making an appointment. Co-payments are due at the time of the appointment.

If you have a high deductible insurance plan or are responsible for co-insurance, you will be required to present a valid credit/debit or HSA card to be charged after your insurance has processed. A credit/debit /HSA card authorization form will be completed and signed prior to being seen by the doctors.

You are responsible for any unpaid balance not covered by insurance and will be charged accordingly.

Insurance coverage and benefit questions should be directed to your health insurance carrier.
