How To Schedule Appointments With Us
We understand that your time is valuable, and we're committed to respecting your schedule by making every effort to see you promptly at your appointed time.
To help us maintain this standard of service for all our patients, we kindly ask that you provide as much advance notice as possible if you're running late or need to cancel your appointment. When you give us early notice of a cancellation, it allows us to offer that time slot to another patient who may be in need of urgent care.
This mutual respect for time helps us serve our entire patient community more effectively while ensuring you receive the dedicated attention you deserve during your visit.
WELCOMING NEW & RETURNING PATIENTS
What To Bring To My Appointment
If you are a first-time patient, please plan on arriving early to complete the patient registration forms. Or, you can click on the "Forms for your First Appointment" tab, complete and print out all the forms prior to your arrival. Please bring the following items to the appointment:
- Valid Picture ID and Insurance card(s)
- Referral forms (if required)
- List of all prescription and over-the-counter medications that you are currently taking
- Any required co-payment.
- Please be aware of your health insurance deductible/coinsurance as you may be required to present a valid credit card.
- Pertinent medical and surgical history information
- Any recent X-rays/MRI
